Story Community
For PHP and IOP Treatment Leaders
What Happens to Your Clients After Discharge Determines Your Program's Real Outcomes
Story Community is the structured post-program environment where clients continue the identity work your curriculum started, led by trained Story Champions, organized around the same 6-stage framework, and built to prevent the isolation that drives relapse.
Most treatment programs end without a continuity structure. Clients leave with skills and no community. Story Community closes that gap.
It is not a social network. It is a facilitated, rhythm-based environment with weekly live sessions, small-group Story Circles, daily structured practices, and Champion-led mentorship, all running on the same framework your clinical team delivered inside your program.
Your clients already speak the language. Story Community is where they keep using it.
Why The STORY Community?
WEEKLY ANCHOR SESSIONS
Champion-led live sessions every week, same day, same time. Teaching, facilitated story sharing, and one clear practice. Clients know exactly when and where to show up.
DAILY STRUCTURED PRACTICES
Daily prompts and reflections organized around a monthly theme drawn from the Story of Self arc. Clients stay active in the framework between live sessions, not just when they feel like it.
CHAMPION MENTORSHIP
Trained Story Champions, not peer volunteers, facilitate the community and mentor clients through continued narrative work. Quality and consistency are built into the model.
STORY CIRCLES
Clients are placed in small pods of 6 to 10 members that meet twice monthly. Research on peer support consistently shows small groups with clear norms create more safety and sustained engagement than open feeds.
STORY CURRICULUM DEVELOPMENT
A structured curriculum track that extends and formalizes the identity work started in your program. Clients progress through it with a cohort, led by trained Champions.
SEPARATE PATHWAYS FOR CLIENTS AND CLINICIANS
Clinical staff and program leaders operate in a separate implementation pathway. Client spaces are protected for psychological safety, the vulnerability the framework requires is preserved.
A Structured Platform Your Clients Access After Discharge

Content Geared Towards Your Journey
Story is an identity curriculum that runs inside Story Community. It takes the framework clients began in your PHP or IOP program and gives it a formal developmental arc, with cohort accountability, Champion-led live calls, weekly peer circles, and a documented identity statement at the end.
This is not a separate product. It is what the curriculum looks like when it continues past discharge with structure and accountability intact.
What clients move through in Story:
- Discover the stages of personal transformation through our storytelling-based curriculum
- Reflect, write, and share your story in a safe, guided space
- Join weekly live calls led by trained Story of Self Champions
- Get personalized feedback and ongoing support in our private group
- Participate in weekly peer circles for deeper connection and accountability
- Craft a new identity and purpose statement rooted in values, not shame
- Become part of a community of people who are choosing growth, not just sobriety
Story Community is available to clients who have completed Story of Self following your program’s discharge pathway. Your Story Champion coordinates client onboarding.
